what happens when the superior oblique is affected?
the eye looks up
what is found in third nerve palsy?
ptosis- drooping of eyelid
eye can move out and down but not in and up
eye brow may be raised to compensate for ptosis
what is found in 4th nerve palsy?
eye looks up as superior oblique muscle is affected
what is found in 6th nerve palsy?
eye can’t look laterally - usually found more medially as the patient wont be able to abduct the eye.
the lateral rectus is involved in 6th nerve palsy
what should you do if someone has 3rd nerve palsy?
check pupillary reflex to see if the parasympathetic is affected or not.
what does the inferior oblique do?
it elevates the eye
the superior oblique depresses it
what test is used to check if someone has 4th nerve palsy?
the cover test
do the H test and when looking medially the affected eye will most likely go up.
what is the presentation of failed pupillary reflex?
pupils enlarged and unreactive
what should you do if the pupillary reflex is absent in patient with suspected 3rd nerve palsy?
no reflex then suspect cerebral artery aneurysm and this is an emergency
this is suspected berry aneurysm
what happens when diabetes affects the 3rd cranial nerve?
diabetes is a microvascular disease and so the nerve is dying because the blood supply is being cut off from inside out.
these conditions therefore usually spare the parasympathetic nerves which are sitting on the periphery of the nerve.
