Eye meds Flashcards
How do prostaglandins work?
Increase uveoscleral outflow
Which medications can change eye colour
Prostaglandins analogues
e.g. latanoprost
How do beta blockers work
Reduce aqueous secretion
Acetazolamide
Carbonic anhydrase inhibitor
Dorzolamide
Carbonic anhydrase inhibitors
Side effects of carbonic anhydrase inhibitors
Peri-oral tingling
Parasthesia
Renal calculi
Which drug can cause peri-oral tingling, peristhesia and renal calculi?
Acetazolomide
Acetazolomide and dorzolamide - which one can be taken orally and which is given in topical form?
Acetazolomide is given oral
Dorzolamide is given in topical form
How do carbonic anhydrase inhibitors work?
Decrease aqueous production
How do parasympathomimetics work? e.g. pilocarpine
Increase outflow
Side effects of parasympathomimetics? e.g. pilocarpine
Pupil constriction - pain
Dimming of vision
Problems at night
Which medications can cause pupil constriction (pain), dimming of vision and problems at night?
Parasympathomimetics e.g. pilocarpine
Pros and cons of trabeculectomy?
Surgery more effective and better compliance
But surgery increases progression of cataract
Surgery treatment for acute closure glaucoma
Peripheral iridotomy - surgical or laser
Treatment for episcleritis
Topical lubricants and topical NSAIDs
Treatment for scleritis
Oral NSAIDs, may require systemic immunosuppresion
Treatment for anterior uveitis
Topical steroids - hourly with gradual taper
Topical mydriatic
Treatment for bacterial corneal ulcer
Antibiotics, e.g. ofloxacin hourly
Treatment for herpetic corneal ulcer
Aciclovir 5 days
Treatment for acute angle closure glaucoma
- 500mg IV acetazolomide STAT
- topical antihypertensives
- topical steroids
- pilocarpine once IOP <50mmHg
- YAG laser peripheral iridotomy
Treatment for ARMD
- anti-VEGF (e.g. lucentis [ramibizumab])
- monoclonal antibodies
- intravitreal injections
Treatment for optic neuritis
IV steroids may hasten recovery but will not affect final visual acuity.
Oral steroids may worsen outcome.
May be some role for B interferon.
Bad adverse effect of mydriatic drops?
Can be a precipitant of acute angle closure glaucoma
Investigation for macular degeneration
Optical coherence tomography
If there is neovascularisation - do fluorescein angiogrpahy
