What is the ideal extraction pattern for mild to moderate crowding in a class I case?
All 5’s/second premolars – taken because not as much space is required. Will finish in Class I.
What is the ideal extraction pattern for moderate to severe Class I cases?
All 4’s/first premolars – if more space is required. Finish in class I.
What is the ideal extraction pattern for mild to moderate crowding in a class II case with a well aligned lower arch?
Upper 4’s/first premolars taken – since to reduce an overjet more space is required anteriorly – finish in a full unit class II molar relationship
What is the idea extraction pattern in a class II cases with moderate to severe crowding?
Upper 4’s/lower 5’s – to reduce overjet, more space required anteriorly. In skeletal II patients the lower posterior segment is brought forward due to not wanting them more class II by retracting the lower teeth.
OR:
All 4’s in moderate crowding in lower arch – lower 5’s
Severe crowding in lower arch 4’s
Both will finish in full unit class I molar relationship, balancing and compensating extractions
What is the ideal extraction pattern for Class III cases?
Due to lower teeth being more prominent, anterior teeth are always considered for extraction to help bring the lower teeth posterior.
Three options: upper 5’s, lower 4’s

What are balancing and compensating extractions?
Balancing extractions are teeth extracted either side of the arc, preventing the dental midline from shifting around to one side of the arch.
Compensating extractions are teeth extracted in the opposing arch to manage occlusion, if one side has a poor prognosis, the tooth on the opposing side will be extracted to compensate.
