Respiratory examination: things to look for from the end of the bed
Cyanosis and respiratory rate.
Chest: scars and deformities.
Cachexia
Medications and aids at the bedside.
Respiratory causes of clubbing.
Chronic cyanosis
supurative lung disease (e.g. TB, cystic fibrosis),
bronchial carcinoma and mesothelioma,
fibrosing alveolitis.
pain: exploring symptom
Site Onset Character Radiation Associated symptoms Timing Exacerbrating or relieving factors Severity.
Abdominal pain: groupings of differential diagnoses.
GI, urological, gynaecological and other random ones.
Abdominal pain: GI causes.
Appendicitis, gallstones, pancreatitis, gastritis/peptic ulcer, diverticulitis, bowel obstruction.
Resp exam: sound of dullness
Collapse or consolidation
Resp exam: stony dullness
Effusion
Resp exam: hyper-resonant
Pneumothorax
Resp: bronchial chest sounds
Consolidation
Resp: crackles
Pulmonary edema, fibrosis or infection
Resp: wheeze
Asthma, COPD
Common symptoms of respiratory disease
Cough, sputum, chest pain, breathlessness, haemoptysis and wheeze.
Resp: persistent cough with few other symptoms
Post-nasal drip, smoker’s cough (if changes, red flag for cancer)
Resp: cough at night, on waking and exercise
