lactulose class
hyperosmotic laxative
lactulose indication
reduction of ammonia absorption in hepatic encephalopathy
lactulose moa
reduces blood ammonia levels by converting ammonia to ammonium
lactulose route
PO or enema/rectal
lactulose can be given
to titrate by number of stools or by ammonia levels. Not just given for high ammonia levels – MUST have signs/symptoms of encephalopathy
lactulose considerations
make sure patient is NOT hypokalemic
rifaximin
second line for encephalopathy if lactulose isn’t working
rifaximin moa
inhibits bacterial RNA synthesis by binding to bacterial DNA - initially used as an antibiotic for GI infections
rifaximin can be given
as preventitive, depends on HCP
rifaximin route
given PO
rifaximin side effects - 8
peripheral edema, nausea, ascites, dizziness, fatigue, pruritis, skin rash, abdominal pain,
rifaximin associated with
an increased risk of C diff - monitor for C diff
