General ROS - 6
Usual weight, recent weight change, fatigue, fever, sleep patterns, or insomnia.
Skin ROS - 7
Rashes, lumps, lesions, itching, dryness, color change, changes in hair or nails.
Head ROS - 2
Headaches, head injury.
Eyes ROS - 12
Vision, glasses or contact lenses, last eye exam, pain, redness, excessive tearing, double vision, spots, specks, flashing lights, glaucoma, cataracts.
Ears ROS - 6
Hearing acuity, tinnitus, vertigo, earaches, infection, and discharge.
Nose/Sinuses ROS - 7
Frequent colds, nasal stuffiness, discharge, itching, hay fever, nosebleeds, and sinus trouble.
Mouth/Throat ROS - 6
Condition of teeth and gums, bleeding gums, last dental examination, sore tongue, frequent sore throats, hoarseness.
Neck ROS - 6
Lumps in the neck, swollen glands, goiter, pain or stiffness in the neck.
Breasts/Axilla ROS - 4
Lumps, pain or discomfort, nipple discharge, self-examination.
Respiratory ROS - 11
Cough, sputum (color, quantity), dyspnea, hemoptysis, wheezing, asthma, bronchitis, emphysema, pneumonia, tuberculosis, pleurisy, and last chest Xray.
Cardiac ROS - 12
Heart failure, high BP, rheumatic fever, heart murmurs, chest pain or discomfort, palpitations, dyspnea, orthopnea, paroxysmal nocturnal dyspnea, edema, past EKGs or other heart tests.
Gastrointestinal ROS - 23
Trouble swallowing, heartburn, change in appetite, nausea, vomiting, regurgitation, vomiting of blood, indigestion, frequency of bowel movements, color and size of stools, change in bowel habits, rectal bleeding, black tarry stools, hemorrhoids, constipation, diarrhea, abdominal pain, food intolerance, excessive belching or passing of gas, jaundice, liver or galbladder problems, hepatitis.
Genitourinary ROS - 11
Frequency of urination, polyuria, nocturia, burning or pain on urination, hematuria, urgency, reduced caliber r force of stream, hesitancy, incontinence, urinary retention, stones.
Genitourinary ROS for males
Hernias, penile discharge or lesions, testicular pain or masses, STIs and their treatments, exposure to HIV, precautions against HIV and STIs.
Sexual interest, orientation, function, satisfaction and problems, method of contraception.
Genitourinary ROS for females
Age at menarche, regularity, frequency and duration of menses, amount of bleeding, bleeding between periods or after intercourse, LMP, dysmenorrhea, premenstrual tension.
If applicable - Age at menopause, menopausal symptoms, postmenopausal bleeding, vaginal discharge, itching, lesions, lumps, STIs, and their treatments, exposure to HIV, precaution against HIV and STDs.
Sexual interest, orientation, function, satisfaction and problems, including dypareunia.
Peripheral Vascular ROS - 5
intermittent claudication, leg cramps, varicose veins, clots in the veins, Raynaud’s disease
Musculoskeletal ROS - 16
Muscle or joint pain, stiffness, arthritis, gout, backache (if any are present, tell teh location and associated symptoms, ex: swelling, redness, pain, tenderness, stiffness, weakness, limitation of motion or activity).
Neurologic ROS - 9
Fainting, blackouts, seizures, weakness, paralysis, numbness, tingling, tremors or other involuntary movements, change in attention span.
Psychiatric ROS - 5
Nervousness, tension, mood changes (including depression), any suicidal ideation or attempts, changes in memory.
Hematologic ROS - 3
Anemia, easy bruising or bleeding, past transfusions, and transfusion reaction.
Endocrine ROS - 6
Thyroid trouble, heat or cold intolerance, excessive sweating, diabetes, excessive thirst or hunger, polyuria.
Glaucoma
Damage to the optic nerve usually due to excessively high intraocular pressure.
Cataracts
Clouding of the normally clear lens of the eye.
Hearing acuity
The clarity or clearness of hearing, a measure of how well a person hears.
