What is evaluation?
Determines whether a patient’s condition or well-being improved after nursing interventions were delivered
What are outcomes of nursing practice?
The measurable conditions of patient, family, or community status; behavior; or perception
Which should be examined when evaluating?
Evaluate the interventions and outcomes in the areas of:
- health promotion
- prevention of illness and injury
- alleviation of suffering
What are evaluative measures?
Similar to and often the same assessment skills and techniques you perform during a patient assessment
Used following nursing interventions to make decisions about a patient’s status and progress
Two important things to evaluate during evaluation
Patient’s behavior
Self-management of the patient
What is the first line of defense for our patients?
Early detection
How should we evaluate the degree of success in achieving outcomes of care?
Objectively
Must have open mind
Actively pursue truth
Be patient and confident
Engage in self-reflection
Two ways to revise the care plan
Discontinue the care plan (if all goals have been met)
Modify a care plan (if it’s only been partially met)
When is patient-centered care achieved?
Only when a patient and family are actively involved in the evaluation process
What does successful collaboration involve?
Interactions in which professionals work together cooperatively with shared responsibility and interdependence toward achieving patient outcomes
