Estimation pression artère pulmonaire

A

-Vélocité max IT pour PAPS. PAPS = 4X vélocité max IT ^2 (mesuré en fin expi) + POD
En absence obstruction CCVD ou sténose pulmonaire
-Pression télédiastolique du jet IP pour PAPD. PAPD = 4 x vélocité télédiastolique IP^2 + POD
En absence sténose tricuspidienne (fin diastole pression VD = pression OD)
-Pression protodiastolique du jet IP pour PAPM. PAP’ = 4xVél max IP ^2 + POD
-PAPM = gradient moyen IT + POD
-PAPM = ⅓ PAPS + ⅔ PAPD
-CIV. PAPS = TAS - 4 x V max CIV ^2 (si shunt G-D) vs PAPS = TAS + 4 x V max CIV ^2 (si shunt D-G)

-OD dilaté (>18 cm2)
-D-shape septum VD en systole
-Dysfonction VD (diminution TAPSE <18mm, S’, FAC <35%, S’ <9.5 cm/s)
-Temps d’accélération pulmonaire <105 ms (PW dans RVOT)
-Notch dans PW RVOT = précapillaire
-Encoche flot pulmonaire en mid-systole (HTN pulm pré-capillaire)
-VCI dilaté et non souple

dp/dt

A

Ventricular dP/dt is the rate of pressure change (dP) with time (dt) during isovolemic contraction of the cardiac ventricles i.e. in the period before the aortic valve and/or pulmonic valve opens, when there is no considerable change in left atrial and or right atrial pressure 1,2. It is a non-invasive method of measuring the contractility of the left ventricle using echocardiography.
