ESTROGENS & PROGESTERONS (progestins) Flashcards
1
Q
NATURAL estrogens: (drug names)
A
- estradiol (estradiol-17 beta, E2)
- PRE-menopausal
- most potent
- produced & secreted by the ovaries
- estrone (E1)
- POST-menopausal
- from conversion of androstenedione in peripheral tissues
- estriol (E3)
- ass. with PREGNANCY
- produced by the placenta
2
Q
SYNTHETIC estrogens: (drug names)
A
steroidal:
- ethinyl estradiol (EE)
- mestranol
- quinestrol (a prodrug of ethinyl estradiol)
- estradiol valerate
non-steroidal:
- diethylstilbestrol
- chlorotrianisene
- methallenestril
- genistein
3
Q
NATURAL & SYNTHETIC estrogens → pharmacokinetics:
A
NATURAL estrogens:
- absorbed trough GIT, skin, mucous
- rapidly metabolized when taken ORALLY
- micronized estradiol → better bioavailability
SYNTHETIC estrogens:
- well absorbed when taken ORALLY
- metabolized more slowly
- prolonged action & higher potency than natural estrogens
4
Q
NATURAL & SYNTHETIC estrogens → indications:
A
- postmenopausal hormone therapy
- primary hypogonadism
- contraception (together with progestogen)
- osteoporosis
- suppression of ovulation in patients with intractable dysmenorrhea
- atrophic vaginitis
- premature menopause
- premature ovarian failure
5
Q
NATURAL & SYNTHETIC estrogens → CONTRAindications:
A
- estrogen-dependent neoplasms (= endometrial)
- high risk of breast cancer
- undiagnosed genital bleeding
- liver disease
- thromboembolic disorders
6
Q
NATURAL & SYNTHETIC estrogens → side effects:
A
- nausea
- breast tenderness
- hyperpigmentation
- migraine headaches
- peripheral edema

- endometrial cancer
- thromboembolism
- hypertension
- MI
- gall bladder disease
7
Q
NATURAL progestogens: (drug names)
A
- progesterone
- hydroxyprogesterone hexanoate
- medroxyprogesterone
8
Q

Q
NATURAL & SYNTHETIC progesterons → indications:
A
- contraception (together with estrogen)
- hormone deficiency
- dysmenorrhea
- endometriosis
- bleeding disorders
- management of infertility
- prevention of threatened miscarriage
10
Q
NATURAL & SYNTHETIC progesterons → side effects:
A
- headache
- depression
- weight gain
- change in libido
- acne & hirsutism (norethindrone, norethindrone acetate, norgestrel, levonorgestrel & other testosterone derivatives)
- increased risk of hyperkalemia (drospirenone)
- increase LDL, decrease HDL
- increase body temp 0,5-1,0°C
- some have mild mineralocorticoid activity
11
Q
gestodene & desogestrel VS. older testosterone derivatives:
A
- less effect on lipids
- less acne, depression & breakthrough bleeding
- higher risk of venous thromboembolism
