anorexiants → drugs included:
- phentermine
2. diethylpropion
anorexiants → mechanism of action:
- ↑ release of norepinephrine, dopamine & serotonin (sympathetic nervous system) → fight or flight mode → ↓ appetite
- result → decrease the appetite
anorexiants → indications:
- recommended for SHORT-term use (<3 months)
2. may lead to tolerance within weeks → weight loss plateaus
anorexiants → CONTRAindications:
- uncontrolled HT
- cardiovascular disease
- arrhythmias
- HF
- stroke
anorexiants → side effects:
- cardiovascular → ↑ BP, tachycardia
- CNS → insomnia, restlessness, alters sexual behavior, hormonal secretion & mood
- others → dry mouth, headache, constipation
lipase inhibitor → drug included:
- orlistat
lipase inhibitor → mechanism of action:
- inhibition of gastric & pancreatic lipase → ↓ absorption of dietary fats
- DECREASE fat absorption by 30%
lipase inhibitor → pharmacokinetics:
3 times/day with major meals (if they contain lipids)
lipase inhibitor → indications:
- BMI > 30
2. BMI > 28 with risk factors
lipase inhibitor → CONTRAindications:
- pregnancy
- chronic malabsorption
- cholestasis
lipase inhibitor → side effects:
(may be decreased if combined with cholestyramine)
- changes in bowel function (due to unabsorbed fat)
- fatty/oily stool, oily spotting
- intestinal gas with discharge
- a feeling of urgency &/or increased no. of bowel movements
- poor bowel control
- kidney & liver TOXICITY (rare)
GLP-1 receptor agonist → drug included:
liraglutide
lipase inhibitor → side effects:
- nausea
- vomiting
- constipation
serotonin agonist → drug included:
- lorcaserin
serotonin agonist → mechanism of action:
- selectively activates 5-HT2c receptors in CNS → DECREASE appetite
- if pat. doesn’t lose at least 5% of bodyweight after 12 weeks → the drug should be discontinued
serotonin agonist → indication:
chronic weight management
serotonin agonist → CONTRAindications:
- severe renal impairment
- history of heart failure → use with caution
- concomitant use with SSRIs, SNRIs, MAOIs & other serotogenic drugs → risk of serotonin syndrome
serotonin agonist → side effects:
- nausea
- constipation
- headache
- dizziness
- dry mouth
- lethargy
- valvulopathy
- rare → mood changes, suicidal ideation
combination drugs → drugs included:
- phentermine + topiramate
2. bupropion + naltrexone
combination drugs → mechanism of action:
- topiramate → anticonvulsant, precise mechanism is unknown
- bupropion → inhibitor of dopamine & norepinephrine uptake
- naltrexone → μ-opiod receptor antagonist (mu)
combination drugs → pharmacokinetics:
topiramate → dosed in steps, escalating dose every 2 weeks
combination drugs → indication:
long-term treating of obesity
combination drugs → CONTRAindications:
- pregnancy (teratogenic)
2. concomitant use of MAOIs → serotonin syndrome
combination drugs → side effects:
phentermine + topiramate:
- ↑ HR
- paresthesias
- suicidal ideation
- cognitive dysfunction
bupropion + naltrexone:
- nausea, vomiting
- constipation
- dry mouth
- potential cardiovascular risk
