Bridging
hip and knee flexion to crook lying stabilisation of feet stabilisation of pelvis and trunk hip extension to neutral with the foot stable - co-contraction at the knee ability to maintain the position
Bridging Exercises
patient in crook lying kneel on either side of their feet facilitate by pushing down through their knees into their heels get them to lift up through their hips sensory input into their affected side
Progression - unaffected foot further away than affected
get them to push out their knees into your hands once hips are lifted
Shifting to Edge of Bed
hip and knee flexion to crook lying lateral shift of legs stablisation of feet stablisation of pelvis and trunk hip extension trunk side flexion eccentric lowering of pelvis neck flexion, trunk flexion and lateral shift of body to realign the body eccentric lowering of upper body
Shifting to EOB Exercises
Teach Bridging - crook lying, therapist pressure though knees, patient lift up pelvis
Teach Rolling - arm out to side, other arm crosses over chest, one leg bent up, therapist facilitates roll at bent leg
Teach Shoulder Action - patient in side lying
kneel facing them
support affected arm
support over scapula
slowly retract and protract at the arm
Rolling
neck rotation to ipsilateral side
move underneath arm out of way - shoulder abduction
upper arm reaches across body - scapular protraction, shoulder horizontal adduction, elbow extension
segmental rotation of trunk
contralateral leg hip flexion, adduction and knee flexion
stability in side lying
Side Lying to Sitting
stability in side lying
lateral neck flexion
abduction of underneath arm
shoulder protraction and elbow extension of upper arm
lateral trunk flexion
hip and knee flexion to bring legs over edge of bed
stability in SOEOB
Supine to Sitting
bridge and shift to edge of bed
neck flexion and rotation in direction of movement
outside leg hip and knee flexion and adduction
inside leg hip and knee flexion and abduction
UL
- outside UL reaching across, inside UL pushing off bed
- bilateral UL protraction and reaching
Supine to Sitting Exercises
roll them onto affected side
lower plinth all the way to ground
bend up unaffected leg
cross affected arm over chest, get them to hold onto it
push through unaffected leg to roll onto affected side
bend hips and knees, bring feet close to EOB
push through plinth with unaffected arm
assist them when upright
Sitting Balance
ability to maintain sufficient postural extension
ability to maintain alignment
ability to maintain COG over BOS for anticipatory movement
ability to maintain COG over BOS for compensatory movement
ability to make protective movements with ULs and LLS to prevent falling
Sitting Balance Exercises
Teaching sitting posture
- sit on affected side
- support at sacrum and sternum
- sit in front of mirror
- facilitate straightening posture
Teaching weight shift and trunk extension
- sit on affected side
- practise shifting them between affected and unaffected side
- imagine lengthening
- once weight shifted, lengthen and straighten trunk
Practising Balance
- take feet off of floor
- head movements
- close eyes
- reaching to the side
- reaching for objects
- practise catching and throwing in sitting
Sit to stand
walk pelvis to front of seat
ability to maintain sitting balance
shifting feet back under BOS
anterior tilt of pelvis with trunk extension
anterior weight shift with DF
extension of hips and knees with trunk extension
standing balance
Sit to Stand exercise
Therapist assistance - therapist stand in front of patient, hands on each others hips, therapist assist into standing, do numerous times
More independent - seated in front of plinth
walk hands over plinth until out of seat
bend elbows and walk hands back until in standing
practise numerous times
Stand to Sit
standing balance
posterior pelvic tilt to unlock knees
anterior pelvis tilt
forward inclination of trunk with extended trunk
forwards movement of knees and psoterior weight shift of pelvis
eccentric quads action to lower the body
sitting balance
Standing balance
ability to maintain sufficient postural extension
ability to maintain alignment
ability to maintain COG over BOS for anticipatory movement
ability to maintain COG over BOS for compensatory movement
ability to make protective extension movements with ULS and LLs to prevent falling
Standing Balance Exercises
reduced BOS - tandem standing, close foot standing
close eyes, turn head
wobble board/cushion
dynamic balance - reaching, catching, movement etc.
