- ABNORMAL READINGS
- ENG. DEPT. PERSONNEL INJURY
- COMPLETE OR PARTIAL POWER FAILURE
- FIRE OR FLOODING
- OPENING SEA STRAINER OR SKIN VALVE
- ENGINE OVERLOAD
- BYPASS SAFETY DEVICE OR AUTO SHUTOFF ALARM
- FLANGE SHIELDING REMOVED
- PUMPING BILGES BY EDUCTOR OR OWS
- FRESH WATER OR LIQUID LOAD WILL AFFECT TRIM
- CHANGE IN PLANT STATUS
- SEWAGE, GREY WATER, OR OWHT REACH 75%

Major equipment - is equipment which if not working properly, will impair the safety, operation, or damage control capability of the ship.
SHALL NOT BE PLACED OOC, WORKED WITHOUT THE KNOWLEDGE AND APPROVAL OF THE EPO.
