Gastric ulcer, ischemic bowel disease, biliary colic (if after eating fatty foods; most likely pain will be in RUQ)
Weight loss
Cancer or gastric ulcer (pain –> decreased appetite)
Tenderness
Pancreatitis
Cough in morning or at night
GERD
Diabetes, bloating
Gastroparesis
History is unremarkable
Non-ulcer dyspepsia
Pain is constant, severe, not related to eating, and radiates to the back
Pancreatitis
Pain radiates into the chest, sore throat, hoarse voice

PPI
Best test to determine etiology of epigastric pain related to ulcer disease
Endoscopy
The MCC of epigastric pain
Non-ulcer dyspepsia (50-90% of all cases)
