1. Small mass separate from testis; can get above it
2. Firm, loculated
3. Transilluminable if large
4. Often in the head of the epididymis
5. May have preceding hx of vasectomy (spermatoceles; bear in mind these do not transilluminate)

1. Conservative
2. Surgical: if painful, very or frequent recurrences
   – aware of compx of damage and fibrosis to epididymis leading to infertility
