Define SHL
A rapid-onset, occurring over a 72 hour period, of a subjective sensation of hearing impairment in one or both ears.
Define SSNHL
A subset of SHL that is a) sensorineural in nature, and b) meets certain audiometric criteria (decreasing in hearing of greater than or equal to 30 decibels, affecting at least 3 consecutive frequencies.
Where is the abnormality in SNHL?
In the cochlea, auditory nerve, or higher aspects of central auditory perception or processing.
What are the most important pathologies to exclude in SSNHL?
Vestibular Schwannoma (acoustic neuroma), stroke and malignancy.
What treatment options are available for SSNHL?
Education
Systemic and topical steroids
Hyperbaric Oxygen Therapy
Middle ear surgery for fistula repair
Observation
Amplification and hearing assistive technology
What Investigations should be ordered on initial evaluation of presumptive SSNHL?
Do audiometry and order MRI.
Don’t do bloods or CT.
