Endurance
the ability to sustain an activity (usually sub maximal) over a period of time. Both cardiovascular and muscular endurance.
Activity tolerance
the amount of activity/exercise that can be sustained before becoming overexerted as measured physiologically and symptomatically, vitals, VO2 max, etc.
how to monitor the patient
assessing vitals pre and post endurance tests
- heart rate
- 02 sat
- BP
- Rate of perceived exertion (RPE)
- Dyspnea scale
RPE - normal borgs scale
total: 6- 20
6-11 light
12- 15 moderate
16- 20 hard
RPE - modified borg scale
total 0-10
0-3: easy
4-6 : moderate/ hard
6-10 : very hard
Dyspnea scale grade 0
no dyspnea. Pt is not troubled with breathlessness
Dyspnea scale grade 1
slight dyspnea. pt toubled by SOB when hurrying on a level surface or walking slightly uphill
Dyspnea scale grade 2
moderate dyspnea. pt walks slower then normal based on age on a level surface due to SOB or has to stop wit breath when walking on a level surface at own pace
Dyspnea scale grade 3
severe dyspnea. pt has to stop for breath after walking 100 yards or after a few minuted on a level surface
Dyspnea scale grade 4
very severe dyspnea. Too breathless to leave home or becomes breathless while dressing or undressing.
6- minute walk test
- requires a hallway 30 meters in length
- test is self paced and sub maximal
- significant improvement should be an increase in 50 meters from first test to midterm test or final test
- norms based on age and gender
2 minute walk test
- used with pts that cannot tolerate a 6 MWT
- same instructions as a 6MWT
- 5.5 meters is a meaningful change
2 minute step test
- associated with the ability to perform lifestyle tasks such as walking and climbing stairs
- this is an alternate test if there is not sufficient space to conduct the 6 MWT
- patient is permitted to use a wall or chair for support
- patient must lift knee to the level between their patella and ASIS
- count the number of times the right knee is raised to the mark on the wall
30 sec chair raise
- chair height 17” placed against the wall for stability
- starting postion: sitting in the middle of the chair, back straight, arms crossed over chest and feet flat on floor
- Take vitals
- demonstrate
- have pt preform
- take post vitals
5 times sit to stand
- same as the 30 secs chair raise instructions but you have the pt do 5 and quickly as possible and time the pt
