Q

Prolactin over secretion and clinical presentation

A

A relatively common condition
Usually due to a pituitary tumour secreting prolactin (prolactinoma)
Clinical presentation
Galactorrhoea (breast milk production)
Amenorrhoea in women and sexual dysfunction in men
Headaches and visual field problems in large tumours

Q

What might mildly raised prolactin be due to?

A

Sexual intercourse
Nipple stimulation
Stress
Large number of drugs (including antipsychotics and antidepressants)
Non-functioning pituitary tumour (compressing the hypothalamus and interfering with the inhibitory effect on prolactin secretion)

Q

What does growth hormone excess result in children?

A

Excessive growth spurt and increased size of feet and hands

If left untreated growth hormone excess leads to gigantism, the most serious consequence of the disease

Acne
Hirsuitism
Fat redistribution
Truncal obesity
Thin extremities
Skin abnormalities
Thin skin and easy bruising

Q

Breast cancer

A

There are two versions of the oestrogen receptor, ERa and ERb
Healthy breast tissue normally expresses more ERB than ERa
Some type of breast cancer strongly express ERa
Oestrogen promotes rapid division of ERa- positive breast cancer cells
ER-positive breast cancers (ER+) can be detected by staining sections of tumour biopsies with antibodies against the oestrogen receptor
The growth and spread of these cancers can be inhibited by administering anti-oestrogen drugs such as tamoxifen

Q

What is the examination for thyroid disease?

A
Signs of thyrotoxicosis:
Hand tremor
Increased sweating
Fast pulse
Inspection of the thyroid:
Enlarged (usually)
Smooth: Grave’s disease
Nodular : toxic nodules
Tender: thyroid inflammation
Swelling around the eyes
Protrusion of the eyeball - proptosis
Paralysis of eye muscles
17
Q

What does hormonal under-secretion in children and adults cause?

A

Children- failure of growth
Adults - tiredness, depression or asymptomatic
Treatment: growth hormone replacement

18
Q

What might steroid under-secretion be due too?

Whats its clinical presentation?

A
May be due to:
Adrenal failure
Pituitary failure
Clinical presentation:
Failure to grow in children
Severe tiredness
Dizziness due to low blood pressure
Abdominal pain, vomiting and diarrhoea
19
Q

Primary hypothyroidism

A

thyroid failure and inability to produce thyroid hormones

20
Q

Secondary hypothyroidism

A

failure to produce TSH

21
Q

Symptoms of hypothyroidism

A
Weakness and dry skin
Sensation of cold and decreased sweating
Impaired memory
Constipation
Weight gain
Hair loss
