How do you diagnose endocarditis?
3 sets of blood cultures with a short period of time inbetween. You should also perform serology
What causes indolent endocarditis?
Strep Viridans, found in the mouth, often occurs after dental procedures or with poor oral hygeine
What causes acute endocarditis?
Staph Aureus, found on the skin, often occurs in IV Drug users.
How would you treat indolent endocarditis?
Amoxicillin/Vancomycin(Allergy) and low dose Gentamicin
How would you treat acute endocarditis?
IV Flucoxacillin/Vancomycin(Allergy)
What bacteria causes endocarditis in patients with prosthetic heart valves?
Staph aureus or coagulase negative staph
How do you treat prosthetic valve endocarditis?
Vancomycin, Rifampacin and low dose Gentamicin
