Emergency Childbirth Flashcards
What is a anternetal record
standard doc for all preg women (should be in one place and accessible)
When technically does the preg start at
first day of the last menstrural period
what does gravid mean
Number of pregnancies a women has
How does the hx of herpes affect childbirth
If pt has active general lesions at the time of delivery, the baby must be delivered via Caesarian section because the baby could be transmitted to the disease
how does mother anxiety/depression affect childbirth
mothers w clinical anxiety, family issues, depression are cared w for an extra 5 days in hospital w extra sleep at night and an assurance that the baby is latching properly before they go home
rubella and childbirth
mothers that have been exposed to rubella during preg w/oo immunization are at risk of passing the disease on to the baby
when are teratogens usually not effective yet
During first 10-14 days post conception the embryo is not at sig risk of being affected
-After 2 weeks the fetus is at risk
What is labour
regular rhythmic contractions of the uterus that change the cervix (softens/enlagres)
-you can have contractions but if they don’t change the cervix its a False Labour
What is stage one of childbirth
Latent phase
how long does the latent period last and how dilated is the mother
can last hours and is about 1-2cm dialated
what are the signs of the latent phase
first signs are LBP and irregular contractions
-can generally last up to 12 hrs (+10hrs in subsequent pregs)
What should be done first aid wise in stage one
simple reassurance for the mother and preparation for later stages
(eg get them where they intend to give birth)
for delivery to be imminent at least two of the following must be present
- regular contractions at one-two min intervals
- An urge to push or bear down
- Crowning- top of head is visible in vag
–If all 3 signs are happening, be prepared to deliver the baby within 1/2 hour
what is stage 2 of childbirth called
the active phase
how large does the cervix get during stage 2
cervix goes from 2-3 to 10cm (second stage occurs when it gets to 10cm)
how long does stage 2 typically last
30-2hrs in the first labour
10-60 muns in subsequent
how to deliver the shoulders
once cleared the head, bring the baby down so the sup shoulder can clear symphysis pubis and then deliver bottom shoulder
steps of the birthing process (active phase)
1Head floating/before engagement
1Head pushing down on cervix and the head is flexed so it can fit under the symphysis pubis (most get stuck here)
- further descent, internal rot (called occiput anterior, which is easiest way for baby heaf to fit)
- Complete rot, begin extension (Babs head is putting major pressure on rectum(
- Complete extension (head has cleared the symphysis pubis and is visible)
- Restitution (external rot)- the babys head will restitute one way or the other in alignment w shoulders`
what is stage 3 of childbirth
Delivering the placenta
how long does it take for the placenta to come out
should be kept for inspection
few mins to several hours after
what to do with the cord post delivery
clamp the cord in 2 places midway bw the baby and vagina
-cut cord w sterile scapula
If the baby is not breathing/crying/gasping what should be done
flick the feet hard or vigorously rub the babys back with a towel, if these do not work, begin resuscitation
