Missing Tooth
X over all surfaces in BLACK/BLUE
for missing tooth, not present or congenitally missing
Impacted or Unerupted tooth
Circle all surfaces in RED
Tooth to be extracted
charted as either a RED diagonal line over all surfaces (or as two parallel lines through the tooth)
Class 1 Caries / Restoration
(posterior occlusal, anterior lingual)
-Chart in RED if it is to be completed (solid fill for amalgam, outlined if composite) on occlusal surface
- chart in BLUE/BLACK once completed
Class 2 Caries / Restoration
(posterior involving interproximal)
-Chart in RED if it is to completed (solid fill for amalgam, outlined if composite)
- chart in BLUE/BLACK once completed
Class 3 Caries / Restoration
(anterior tooth no incisal edge)
-Chart in RED if it is to completed (solid fill for amalgam, outlined if composite)

Class 4 Caries / Restoration
(anterior tooth with incisal edge)

Gingival Hyperplasia
- Charted in RED, drawn on the tooth to depict the level of the gingival tissue
Abrasion
- Charted in RED with double straight lines on cervical 1/3 of tooth
Drifting
-Charted in RED, an arrow is placed in the direction in which the tooth is drifting
(arrow below tip of root on facial surface)
Diastema
-Charted in RED with 2 vertical lines between the teeth
Fractured Tooth/Root
Charted in RED where the fracture occurred
Denture
-Charted in RED if to be completed
-Charted in BLUE/BLACK if already fabricated
Furcation
Charted in RED
-Triangle at crotch of tooth
Class 1 = ^
Class 2 = outlined triangle
Class 3 = filled in triangle
Recession
-Charted in RED as a curved line on the tooth Measurement recorded beside tooth in MM
Probing Depths
- recorded in blue or black ink for pockets under 3mm and under in depth. Any pocket 4mm or over is recorded in RED
Bleeding points
If bleeding is noted during probing, it is noted with a RED dot above the measurement
Suppuration (pus)
Noted as RED encircled dot
Mobility
Recorded in RED in Roman Numerals above or below the root of the tooth
N=normal
1=slight mobility, greater than normal
2=moderate mobility, greater than 1mm of displacement
3= severe mobility, tooth moves vertically and is depressible in the tooth socket (Robinson, 2024).
Mobility can be charted at the root of the tooth or there may be a specific line on the periodontal chart.
