Order of aV leads on EKG
RLF
Axis on EKG
Upright QRS in I and aVF = Normal
Upright QRS in Lead I and downward in aVF = left-axis deviation
Downward QRS in I and aVF = right axis- deviation
EKG lead groupings
V1-V2 = septal leads II, III, aVF = inferior leads I and aVL = lateral leads V3-V4 = anterior leads V5-V6 = lateral leads
Normal intervals
PR = 120-200ms
QRS < 120
AV block EKG
PR > 200 or P with no QRS afterwards
LBBB EKG
QRS > 120
No R wave in V1
Wide, tall R waves in I, V5, V6
RBBB EKG
QRS > 120
RSR’ complex ie rabbit ears
qR or R morphology with a wide R wave in V1
QRS pattern with wide S wave in I, V5, V6
Long QT syndrome on EKG
QTc>440
Congenital disorder that predisposes to ventricular tachyarrhythmias
T wave abnormalities with ischemia and infarction
Ischemia: new inverted T waves; poor precordial R wave progression
Transmural infarct: significant Q waves (>40 mse); ST elevations with T wave inversions
LVH EKG
S wave in V1 + S wave in V5 > 35mm
RVH EKG
Right axis deviation + R wave in V1 that is > 7mm
