JVD
JVP > 7cm above sternal angle
Kussmaul’s sign
Increased JVD upon respiration
-can signify right heart MI, post-op tamponade, restrictive pericarditis, tricuspid regurgitation
Harsh, systolic ejection murmur that radiates to the carotids
Aortic stenosis
Holosystolic murmur that radiates to the axilla or to the carotids
Mitral regurgitation
Midsystolic or late systolic murmur with preceding click
MVP
Early decrescendo diastolic murmur
Aortic regurgitation
Mid-to late diastolic, low-pitched murmur
Mitral stenosis
S3 gallop
Dilated cardiomyopathy (sloshing in) ie a floppy ventricle
Mitral valve disease
Often normal in younger patients or in high-output states
S4 gallop
A stiff heart LV hypertrophy HTN Diastolic dysfunction (a stiff ventricle) Aortic stenosis
Increased peripheral pulses
Compensated aortic regurgitation, coarctation, PDA
Pulsus paradoxus
Decreased SBP with inspiration (it normally increases) pericardial tamponade asthma COPD Tension pneumothorax
Pulsus alterans
Alternating weak and strong pulses
Cardiac tamponade
Imaired LV systolic function
Pulsus parvus et tardus
Weak and delayed carotid upstroke
Aortic stenosis
Single or paradoxical split S2
