Definition of Anasarca
Massive generalized edema
How is edema and swelling different?
Swelling is a transient event
Pitting edema
Associated w/ underlying organ dysfunction (heart, kidneys, liver, etc.)
Non-pitting edema is associated with what kinds of diseases?
Metabolic diseases, like thyroid (Myxedema) or lymphatic system disease
What kind of edema can be associated with warm weather?
Non-pitting edema
What causes Lipidemia?
Fat cells grow and proliferate, which leads to fluid retention around the cells
What can Lymphedema be difficult to discern from?
Morbid obesity
In what diseases can a patient be morbidly obese and have lymphadema? (3)
CHF
Nephrotic syndrome
Severe liver disease
What is the advanced form of CHF?
Pulmonary edema
DDx for BL edema - 6 systems
CVS: HF, constrictive pericarditis, pulmonary HTN
Renal: advanced kidney disease, nephrotic syndrome
Liver: cirrhosis
Hematologic: anemia
GI: nutritional deficiency
Endocrine: Myxedema
What meds can cause edema? (5)
Anti-depressants (MAO inhibitors) Anti-hypertensives (CCBs, direct vasodilators, beta-blockers) Hormones NSAIDs Thiazolidinediones
Aside from the same causes of BL edema, what can cause UL edema? (3)
Lymphedema secondary to neoplasm, surgery, radiation, etc.
Cellulitis
Baker cyst
Diagnostic testing to evaluate edema (4)
Labs: CMP, CBC, TSH
CXR-PA and lat.
ECG
Echo
What is preferred: BNP or ANP levels?
BNP since it has a wider range of concentrations than ANP.
What produces BNP?
Ventricles
