When considering Edema. We need to think about ?
Blockages - DVT.
Leaks - Aneurysm that can rapture.
Overflow - CHF. The inability for the hear to pump blood causing swelling legs.
DVT in the Lower leg can cause?
Edema that is also red or discoloration, warmer the the opposite limb and painful.
DVT should be evaluated to avoid?
Leg cramps and/or decloth.
What medical management can help with Edema?
Diuretics Beta blockers Immobilization When it is safe to apply compression, Elevate the extremities.
What could diuretics due ?
Lowe the BP. Additionally for CHF - helps pull out fluid from legs and lungs.
What does beta blockers do?
Controls BP
When is it safe to apply compression ?
When there sis not suspicion of DVT or infection, or limp compromise blood supply
When should we avoid elevating BLE?
When there is a heart condition such as CHF. This can cause fluid build up around the heart and lungs.
What is Lymph system?
It is a part of immune system.
Cleans viruses, bacteria and other debris in the body.
How does lumpy system work?
No pump
Works with pressure
Lymph Nodes if damages?
The do not regrow nor regenerate.
Cervical nodes drain ?
Left face/neck
Axillary Nodes grain?
RUE/ Chest
Inguinal nodes drain?
RLE

A pressure. This is the exchange between vessels/arteries and lymphatic system
What can help the exchange of fluids in the thoracic duct?
Deep breathing.
What are the Non-cardiac causes of Edema?
Injury Inflammation Medications Immobility Kidney disease Liver Disease Edema can also be cause by the use of Steroids.

Acute
Subacute

Soft
Easy movable
easy Pitting
Quick rebound and to resolve

Feels like patty
Pitting is slow
Rebounds Slow
Difficult to move around
Classification of Edema, Chronic
Hard, difficult to treat or change
Hard to do anything
Stages of Lymphedema, what are they?
0 - Subclinical
1 - Notable, still reversible
2 - Notable, not easily reversible
3 - Severe, other skin changes
0 - Subclinical
At risk for lymphedema due to surgery. No symptoms
1 - Notable, still reversible
Similar to stage one. Edema present
2 - Notable, not easily reversible
Subacute like.
3 - Severe, other skin changes
Non-reversible
What are secondary complication of chronic Edema?
Chronic hardening of the skin. Infections Immobility Loss of function Emotional /Social impact
What are some causes of localized edema ?
Inflammation after an injury, localized allergic reaction, immobility of the limb, dependent positioning, venous disease.
What are some contraindications to compression wraps or MEM?
The presence of blood cloth (DVT) or hematoma, infections, inflammations, cancer, arterial disease, severe cardiac/liver/kidney disease, acute wound or injury, and lymphedema.
