What are types of eczema?
Contact dermatitis: type 4 hypersensitivity reaction, either irritant (e.g. soap, repetitive handwashing), allergic (cobalt, nickel, fragrance)
Atopic: IgE mediated type 1, associated with asthma and hayfever.
Seborrhoeic: due to yeast on the face e.g. scalp, face, eyebrows.
Venous: due to underlying venous pathology e.g. venous ulcer.
What are complications of eczema?
Staph aureus infection, herpes simplex.
What is testing for eczema?
Atopic: skin prick. IgE levels.
Contact: patch test.
What is presentation of venous dermatitis?
Long history of leg swelling and discolouration with dryness. Can get ulceration.
Peripheral pulses are present, likely to be varicosities.
What predisposes to venous ulcers?
Hypertension
Obesity
Multiple pregnancies putting pressure on the venous system.
Amlodipine can cause peripheral oedema.
What investigation in potential venous ulcer?
Ankle brachial pressure index >0.8 suggesting higher pressure in ankles ?
