  • high flow O2 10-15L via a non re-breath mask
  • check for fogging

Circulation

A
  • IV access 2x14g cannula
  • bloods- FBC, G+S, LFT, U&Es, coag, cross match
  • fluids (restricted- increased risk of pulmonary oedema)- up to 1L crystalloid fluid- 1ml/kg/hr
  • urinary catheter- 0.5mls/kg/hr
  • obs every 3-5 mins
  • fluid balance chart

Immediate Care

A
  • commence magnesium sulphate loading dose- 4g IV over 5 mins
  • infusion rate- 1g/hr for 24 hours
  • ensure continuous O2 sats monitoring
  • consider fetal distress- expedite birth if needed and alert NICU

Ongoing Care

A
  • consider MgSO4 toxicity- stop infusion
  • monitor for signs- less than 12 resps
  • antidote- calcium gluconate 1g in 10mls 1ml/min
  • obstetric review for antihypertensive
  • 4 hourly senior review
  • alert team members- possible current seizures
