Left bundle branch block
caused by htn, hyperkalemia, dig toxicity, aortic stenosis, extensive MI
Right BBB
caused by CAD*, PE, myocarditis, cardiomyopathy, right ventricular hypertrophy
3rd degree heart block
cause: MI, dig toxicity
treatment: check pt, check electrolytes, meds, TCP, TIP, dopamine & epi gtt, PPM if needed
Mobitz II
Cause: anteroseptal MI
Treat: watch for advancing black, TCP, TIP, PPM
Mobitz I/Wenkebach
cause: ischemia at AV node, meds, dig toxicity, post-cardiac cath if through femoral
Treat: meds, watch for advancing block, check electrolytes
First degree av block
cause: congenital, meds, MI, beta blockers post MI
treat: check meds, electrolytes, no trestment unless symptomatic
Torsades de pointes
Cause: R on T, pronged QT, electrolyte imbalances, hypothermia, non sedating antihistamines, severe brady
Treat: code blue, cpr, Magsulf, defib
Vfib
Cause: MI, R on T
Treatment: epi, CPR, defib
Vtach
Treatment: stable: notify md, watch for hemodynamic compromise, amiodarone
Unstable: cardiovert (if pulse), defib if no pulse
Accelerated idioventricular
60-100 bpm
Treat: atropine if symptomatic
Idioventricular rhythm
40bpm or less
Prepare for tcp/tip, dopamine gtt, epi gtt
PVC
observe frequency (3 or more-problem), check meds, check electrolytes
