Rate if regular rhythm?
300/R-R
Rate if irregular rhythm?
Count R waves in 10 seconds, multiply by 6.
Axis Deviation?
Line up left thumb with I, Right with aVF.
Normal Axis
L + R thumbs line up (+ve deflections)
Left Axis Deviation?
L thumb lines up (+ve), R doesn’t (-ve)
Right Axis Deviation?
L thumb doesn’t (-ve), R does (+ve)
What do V1, V2 show?
R ventricle
What do V3, 4 show?
Septum
What to do V5, 6 show?
L side of heart
QRS normal length?
≤ 0.12 seconds (2-3 small squares)
Bundle Branch Block?
QRS in V1 + V6.
LBBB: ‘W’ in V1, ‘M’ in V6.
RBBB: ‘M’ in V1, ‘W’ in V6.
Heart Block?
PR interval: ‘‘Deterioration of romantic relationship between P and QRS’’
Type 1: consistently prolonged.
Morbitz Type 1: progressively gets longer
Morbitz Type 2: normal, occasionally missing QRS
Complete: No association between P + QRS
Ventricular Hypertrophy?
V1 + V5/6
RVH: V1 dominant R wave (+other signs covered later)
LVH: V5/6 >5 big big squares tall
ST elevation basics?
Elevation, Depression + leads
Elevated in a few leads: MI
Depressed: Ischaemia OR posterior MI
Elevated in every lead: Pericarditis/Tamponade
What do II, III and aVF show?
Inferior side of heart.
