ECG Flashcards
STEMI/NSTEMI + bradycardia indicates
The Bezold-Jarisch reflex - inhibitory reflex of bradycardia, vasodilation, and hypotension resulting from stimulation of cardiac receptors.
When may the BJR occur?
In acute inferoposterior myocardial infarction
Function of the Bezold-Jarisch reflex
Thought that the cardiodepressor Bezold-Jarisch reflex (BJR) acts as a protective reflex to vasodilate the coronary arteries in the setting of inferoposterior infarction associated with coronary vasospasm.
Subtle sign of inferior STEMI
ST depression in aVL
aVL is the only lead truly reciprocal to the inferior wall, as it is the only lead facing the superior part of the ventricle.
91% of “subtle” inferior STEMIs that do not meet STEMI criteria but show occlusion on PCI demonstrate ST depression in aVL
Leads for lateral MI
V5, V6, aVL, I
Leads for anterior MI
V3, V4
Leads for septal MI
V1, V2
Leads for inferior MI
II, III, aVF
