How does Primaxin work?
Extremely broad spectrum
With Imipenem and Cilastin
Imipenem/carbapenem works by inhibiting PBP but is inhibited in the kidney.
Cilastin inactivates the dipeptidases in the kidney.
What are the indications for primaxin use?
Bacterial septicemia
Bone/joint infections
Skin infections
Polymicrobial infections.
What is the dosage for Primaxin?
500 IV q6
Careful for side effect of siezures.
What’s good about Meropenem use?
It can be used as a less toxic/less seizure drug compared to Primaxin.
Has pseudomonas coverage and is synergistic with aminoglycosides against pseudomonas.
Meropenem dose?
500mg IV q 8
Not for MRSA!
What drug interactions are related to Meropenem?
Probenicid increases meropenem levels.
Valproic Acid levels are decreased with meropenem leading to possible seizures although still less than Primaxin.
What is invanz?
Ertapenem to be used IV or IM for complicated skin infections.
Interacts with Valproic acid increasing risk of seizures.
Do carbapenems cross react with penicillins?
Yes as it has a beta lactam ring
Should Tdap be given during pregnancy?
Absolutely should be given during every pregnancy around the 27-36 week mark.
