usually caused by URI - eustachian tube takes on serous fluid and fluid accumulates in the middle ear

A

popping sensation in the ear, fullness in the ear, mild conductive hearing loss, and sometimes pain

What is characteristic on examination of otitis media w/serous effusion?

A

amber fluid behind the eardrum

  • a fluid level, a line between air above and amber fluid below, can be seen on either side of the short process
  • air bubbles are sometimes present but not always

  • s/s: earache, fever, and conductive hearing loss
  • the eardrum reddens, loses its landmarks, and bulges laterally, toward the examiner’s eye
  • sometimes perforation w/discharge of purulent material

Cholesteatoma. What is it? What does it look like? s/s? what would you do?

A
  • Type of chronic otitis media with peeling layers of scaly epithelium
  • s/s: painless otorrhea, ear canal filled with keratin debris or granulation tissue, hearing loss, TM perforation around 90% of the time
  • REFER TO ENT
  • cyst-like mass behind the eardrum caused by proliferation of squamous epithelium
  • examination reveals collection of white granulation tissue with perforation of the TM
