Applicants must also show:

A
  • location will be practicing bona fide pharmacy
  • number of hours each week the pharmacy is open to public
  • sufficient stock of emergency and frequently prescribed drugs
  • registered pharmacist in personal attendance and on duty when practice of pharmacy is conducted
  • pharmacy will be separate from areas containing merchandise not offered for sale under permit (OTC, non drug items)

  • notify the board 20 days before the transfer of controlled substances & submit copy of inventory to DEA with name, address, and DEA number of person drugs are being transferred to
  • remove all legend drugs from stock be either returning to wholesaler/manufacturer, transfer to another pharmacy, destroying in presence of board rep
  • dispose all controlled drugs and legend drugs and submit the licensed premises for inspection by board rep before disposing of any other merchandise
  • remove symbols or words of pharmacy
  • return pharmacy permit for cancellation within 10 days of all legend drugs, controlled drugs, and devices being removed
