dyspepsia
early satiety, post prandial fullness, epigastric pain or burning
GERD
symptoms or complications resulting from the reflux of gastric contents into the esophagus or beyond (oral cavity/lung/larynx)
peptic ulcer disease
circumscribed ulceration of GI mucosa in areas exposed to acid and pepsin
risk factors for GERD
stress poor diet hiatal hernia obesity pregnancy large meals meds that cause sphincter loosening (NSAID, nitrates, CB, anticholinergics) etoh/chocolate/mint smoking asthma
risk factors for PUD
previous ulcers
smoking
ETOH
meds (corticosteroids, NSAIDS, anticoagulants, EERI, spironolactone)
stress
Medical: cirrhosis, COPD, renal failure, celiac, CD
H pylori
why does GERD –> asthma
micro aspiration –> asthma
asthma –> GERD
GERD time course
< 3min, 30-60 min after eating
2X/wk
PUD time course
1-2 hours after eating or empty stomach
rome criteria for IBS
1 day/week for 3 mo
what is normal on the bristol?
3 & 4
tx for IBS
peppermint
fibre (plus or minus)
SSRI’s!
