DVT Mind-maps Flashcards
What is it
A clot that usually develops in one of the deep veins
Usually occurs in the leg
Signs and symptoms
Asymptomatic Pain Oedema Erythema/discolouration Increased temperature of symptomatic leg Engorgement of the surface veins
What is Virchow’s triad
Hyper-coagulability
Endothelial injury/trauma
Stasis of blood flow
Causes of hyper-coagulability
Malignancy Surgery Trauma Oral contraceptive pill Clotting abnormalities
Causes of venous stasis
Immobility e.g. after surgery
Pregnancy
Heart failure
Causes of endothelial trauma
Inflammation
Previous thrombosis
Complications
Pulmonary embolism
Post-thrombotic syndrome
Differential diagnosis
ABC:
A musculoskeletal injury
Baker’s cyst rupture
Cellulitis
Investigations
D-dimer
B-mode venous compression ultrasonography (DVT ABOVE knee)
Investigations to uncover the cause of DVT
Modified Wells Score
Is D-dimer test sensitive or specific
Sensitive but not specific
What is meant by D-dimer test being sensitive but not specific
If the result is negative then the cause is unlikely to be DVT
What is purpose of Modified Wells score
Calculate probability of DVT
Treatment
Anticoagulation therapy:
Unfractionated heparin or a LMW Heparin
Treatment: secondary management of DVT after anticoagulation therapy
Vitamin K antagonist e.g. warfarin
Example of LMWH
Dalteparin
