For a DVPN application to go through it must be approved by an authorising officer of what rank?

What happens if the perpetrator breaches a DVPN?
A constable may arrest the perpetrator without warrant if there are reasonable grounds for believing that he is in breach of the DVPN
What duty do police have when they arrest someone for breaching a DVPN?
Police have a duty to remand in custody to enable the perpetrator to be brought before the magistrates’ court within 24 hours of the time of arrest, or the time of hearing for the DVPO, whichever is the earliest
Do police have a power of entry for a breach of a DVPN?
There is no power of entry to arrest for a breach of the DVPN, although other powers such as PACE s17 may apply if there’s a concern for life or limb.
