Lists the psychiatric diagnosis or diagnoses (for example, major depressive disorder and alcohol dependence) that are generally considered biological in nature. All psychiatric disorders are listed here with the exception of personality disorder and mental retardation.
AXIS I
Lists personality disorders and mental retardation to ensure long-standing issues that may co-occur with the psychiatric diagnosis are considered, such as borderline personality disorder.
AXIS II
Lists any medical conditions the patient may have, which may or may not influence the mental health diagnosis (for example, coronary artery disease or hypothyroidism).
AXIS III
Lists any psychosocial stressors in brief narrative form, such as homeless going through a divorce, or job loss.
AXIS IV
The Global Assessment of Functioning (GAF) score, is noted on this axis. The GAF is rated on a scale of 1 to 100 and indicates the patient’s level of functioning. The higher the score, the higher level of functioning.
AXIS V
