Severe otitis media
A
- Augmentin, if no improvement in 2-3 days - ceftriaxone
- pcn allergy - ceftriaxone; if no improvement in 2-3 days - clindamycin

A
- Amoxicillin, if no improvement in 2-3 days - Aumentin
- non type 1 pcn allergy - azithromycin, ceftriaxone; if no improvement in 2-3 days - ceftriaxone
- type 1 pcn allergy - cefnidir, cefuroxime; if no improvement in 2-3 days - clindamycin

AOE - mild treatment
A
solution of acetic acid or alcohol plus acetic acid

moderate AOE
A
topical cipro (not ototoxic) (can be expensive)

severe OAE
A
if infxn spreads to pinna, oral cipro for adults, keflex for children

necrotizing otitis externa
A
antipseudomonal ear drops (ofloxacin solution) along with cipro - referral to specialist
