CCF and hypertension
What can Calcium antagonists be used for?
Hypertension and angina
What are nitrates used for?
CCF and angina
when can diuretics not be used?
If the patient has gout
When are beta blockers contraindicated?
Asthma
Which drugs are anti-hypertensives?
Thiazide diuretics
Alpha/Beta blockers
ARBs/ACE inhibitors
calcium antagonists
Which drugs are anti-thrombotics?
Antiplatlets
Fibrinolytics
Anti-coagulants
Give examples of antiplatlets
Aspirin, clopidogril
Give examples of Fibrinolytics
tPA or streptokinas
Give examples of anti-coagulants
Riveroxiban or warfarin
Which drugs are anti-cholesterols?
Statins and fibrates
Which drugs are anti-anginas
Nitrates Beta blockers Calcium antagonists Nicorandil Ranolazine Ivabradine
What do diuretics do?
Block Na+ reabsorption
Give an example of a thiazide diuretic
bendrofluazide
what are thiazide diuretics used for?
Hypertension
Give an example of a loop diuretic
Furosemide
What is furosemide used for?
Heart failure
What are the side effects of diuretics?
Tired, hypokalaemia, arrhythmias, diabetes and gout due to increased uritic acid
How do beta blockers work?
Blocks B1 and B2 adrenoreceptors
Give an example of a B1 receptor selective beta blocker
Atenolol
What is Atenolol used for?
Angina, hypertension and herat failure
Give an example of a non-selective beta blocker
Propranol
What is propanol used for?
Thyroxitosis
What are the side effects of beta blockers?
Tired and may worsen HF in the short term
Give an example of a calcium antagonist
Dihydrophines e.g. amlodipine
Veramparil
what is amlodipine used for?
hypertension and angina
What are the side effects of amlodipine?
Ankle oedema
When should verapamil be avoided?
When also using beta blockers
Give an example of an alpha blocker
Doxazosin
What are the side effects of Doxazosin
Postural hypertension
How do ACE inhibitors work?
Prevents the conversion of angiotension I to angiotensin II
Given an example of an ACE inhibitor
Lisinopril
What are the side effects of ACE inhibitors?
Cough, renal dysfunction and angioneurmtic oedema
When should ACE inhibitors and ARBs never be used?
In pregnancy
How do ARBs work?
Block angiotensin II receptors
Give an example of an ARB
Losartan
What are ARBs used for?
Hypertension and heart failure
What are the side effects of ARBs?
Renal dysfunction
Give examples of nitrates
Isosobride and mononitrate
What are the side effects of nitrates?
Headaches and hypotension
How often should be left nitrate free in a day and why?
8 hrs a day to avoid tolerance forming
Give examples of antiplatlet agents
aspirin and clopidogril
What are antiplatlets used for?
Angina, acute MI, TIA and for those who are high risk of CVA
What are the side effects of Anti-platlets?
Haemorrhage and peptic ulcers
How do anticoagulants work?
Prevent formation of a new thrombosis
How should heparin be given?
IV only
What does warfarin do?
Blocks clotting factor 2,7,9 and 10 - used in DVT, pulmonary embolisms and NSTEMIs
How does riveroxiban work?
Factor Xa inhibitor - Xa converts prothrombin II into thrombin
How does dabigatran work?
Thrombin factor IIa inhibitor
How do fibrynolytics work?
Dissolve formed clots
Give examples of fibrinolytics
Streptokinase or tissue plasminogin activators
How do statins work?
Blocks HMG Co reductase
What are statins used for?
Hypercholesterolaemia, diabetes, angina, MI and TIA
What are the side effects of statins?
Myopathy or can cause renal failure
Give an example of a fibrate
Bezafibrate
What should be used for SVTs?
Adenosine
What should be used for ventricular/supraventricular arrhythmias?
Beta blockers, flecainide or amiodarone
What are the side effects of amiodarone?
Phototoxicity, pulmonary fibrosis and thyroid abnormalities
How does digoxin work?
Blocks AV conduction - used in AF
Why is digoxin bad in excess?
Reduces HR and so can cause bradycardia and heart block
What is a side effect of digoxin?
Increases ventricular irritability which produces ventricular arrhythmias
How does digoxin toxicity clinically present?
Nausea and vomiting, yellow vision, bradycardia and ventricular arrhythmias
