Thiopental
induction: 2.5-5mg/kg
Methohexital
induction: 1-2mg/kg
rectal (pedes): 25mg/kg
infusion: 50-150mcg/kg/min
Midazolam
preop: .02-.04mg/kg
PO: .4-.8mg/kg (admin 30 minutes pre induction)
.2mg/kg IV starts to decrease BP and increase HR. CO not altered
postop sedation: .5-4mg loading dose, 1-7mg/hr gtt
paradoxical vocal cord motion: upper airway obstruction with stridor: .5-1mg

antiemetic: 10-15mg IV followed by 10mcg/kg/min infusion
antipruritic: 10mg IV r/t intrathecal opioids, cholestasis
anticonvulsant: 1mg/kg IV

induction dose: .2-.4mg/kg IV
induction dose up to .3mg/kg results in minimal SE.
>.45mg/kg results in SE.
ketamine
adult induction: 1-2.5mg/kg IV
IM: 4-8mg/kg (<10min onset)
10mg/kg PO (10-20min onset)
analgesia at sub anesthetic doses: .2-.5mg/kg (somatic >visceral effects)
.5-1mg/kg prior to surgical incision. may be followed by infusion. has a role in preventing hyperalgesia
dexmedetomidine
TIVA: .5-1mcg/kg bolus over 15 minutes
infusion: .2-.7mcg/kg/hr (up to 24 hours)
4mcg bolus for pain, up to 16mcg
.5-2mcg/kg for pain
scolpalamine
preop: .3-.5mg IV/IM
physostigmine
15-60mcg/kg IV and repeat as required
codeine
15mg oral cough suppressant
60mg=650mg ASA
120mg IM=10mg morphine
meperidine
