describe Sock aid to client
this is a device used to help put socks on independently without having to break any hip precautions
describe
Long handled shoe horn
to client
this is a device much like a regular shoe horn that will help slide your foot into the shoe. it is long to prevent you from having to bend past 90 degrees.
Describe Reacher
to client
It works like an extension of your arm allowing you to reach down and pick things up off the ground without bending or stooping over. it can come in handy while dressing to adjust the shoe on the ground. to use it you just squeeze the handle to grab things, and release handle to let go
Describe dressing stick
to client
this is a handy too while dressing, it has different hooks to help grab clothes without bending. you can use the hook to grab loops of shoes, or pull socks up to straighten them. also handy while putting on pants- you would slide the stick through the pant leg on the injured side. hook gathers pant leg while you bring it to your foot. slide leg in and pull pants up with stick. for other leg, put in pants normally
begin dressing instruction with surgical leg or good leg?
when undressing which leg goes first?
Surgical leg 1st dressing
undressing surgical leg last
List all 3 posterior precautions
layman’s terms
- No bending at the hips than 90 degrees. when sitting in chair that is as far as you can bend. also applies to lifting knees off chair.
- No turning knees or foot inwards
- No crossing legs or feet, best to keep feet and knees slightly apart
list all 3 anterior precautions
in layman’s terms
- No pointing knees or toes outward
- No crossing legs or feet, best to keep feet and knees slightly apart
- cannot bring leg backwards when walking, must keep it in line with your hip or slightly forward
explain how to use sock aid
- Roll opening of sock, and stretch over opening of sock aide
- Pull sock all the way up but not over the knots in rope leaving a little slack in the toes.
DO NOT PUT B/W LEGS! - drop aid on ground holding one handle, or drop whole thing
- use the Reacher to line up with foot sticking toes in and then to pick up the strings.
ANTERIOR THR= put more on inside of foot to avoid external rotation
POSTERIOR THR= put more on outside of foot to avoid internal rotation - holding the strings keep leg straight while pulling on strings to apply the sock.
DO NOT LIFT FOOT OFF GROUND OR RAISE KNEE! - Use dressing stick or reacher to straighten the sock as necessary
Explain how to put shoes on using reacher, shoe horn, and dressing stick.
- pick up shoes with reacher and bring to lap, untie and open them as much as possible
GOOD PLACE TO EXPLAIN CURLY LACES OR BETTER SHOE OPTIONS (slide on shoes, shoes with non slip tread etc) - Put on ground and use reacher to line up with the foot BE MINDFUL OF PRECAUTIONS.(inside of foot for anterior, outside of foot for posterior)
- Straighten leg to get foot off ground, but avoid lifting knee
- holding the tongue of shoe with reacher, slide toes inside.
- slip shoe horn on the inside of the shoe near ankle, then slip it around to the back of heel
- slide foot into the shoe.
- Use reacher to adjust shoe as needed.
- explain dressing stick is used to hook loop on shoe or for adjustment though most people think the reacher is easier.
Full list of steps for skills check
- Introduce self and purpose of meeting
“hello, I’m jenny an OTA student at fox college. I understand you just had a hip replacement so I’m here to teach you how to use some devices to help you with dressing” - verify name and DOB
- Ask about pain level
- define assistive tech
- review all assistive tech devices (Reacher, sock aid, dressing stick, shoe horn)
- Verify surgical site (post/ant, left/right)
- Review hip precautions & ensure the client is comfortable.
- Demonstrate steps of sock aid use
- starting with surgical side first have client use sock aid while watching for precautions.
-ASK ABOUT PAIN AGAIN - Demonstrate putting shoes on (affected extremity first) going over precautions a 2nd time
- Have client complete donning shoes while following precautions using available tools. explain use of elastic laces.
- Ask about pain last time. ask if the client has anymore questions or needs to see anything again. Then thank them, and ensure them that if they have any questions to feel free to reach out.
