Dr. Elggren and Dr. hume lecture Flashcards
The DEJ is formed from the dental papilla and what?
-Dental follicle
What is the matrix protein of dentin?
-Collagen
What gives dentin its characteristic structure?
-Odontoblastic process
Why can dentin and pulp be considered a single biological entity?
-Because the dentinal tubules connect them
What are the functions of the pulp?
- To make dentin (continues throughout life)
- Sensation
- Repair following insults
What zone do you find under the odontoblasts?
-Cell free zone of Weil because it is so thick of nerve processes
What causes the nerve cells to fire?
-When the fluid in the tubules move
How long does it take for a chemical to move from the dentinal tubules to the pulp space?
-A couple of hours
What is the first response from toxins/infection invading dentinal tubules?
-Chronic inflammation
What are the symptoms of chronic inflammation?
- No symptoms
- Silence
As a carious lesions gets worse and worse what type of inflammation do you get?
-Acute inflammation
What are the symptoms of acute inflammation?
-Pain from cytokines that sensitize the nerves
Initially what does that tooth become sensitive to?
-Hot and Cold usually cold first
T/F If you take away the insult the pulp can heal
True
T/F Internal tooth resorption is very common with bacterial insult
False
-It is very uncommon
What are insults to the pulp that can occur during tooth restoration?
- Thermal
- Mechanical
- Chemical
What is the response to thermal, mechanical, and chemical insults to the pulp?
-Pain because of acute inflammation
If you dry cut to close to the pulp what can happen?
-You can kill the pulp
What can cause insults to the pulp months or years after restoration?
-Bacterial micro leakage around restorations
What are the pulpal responses to leaky restorations?
- Chronic inflammation
- Acute foci
- Pulp death if untreated
Do normal teeth have a response to cold?
-Yes
If you get a knee jerk reaction that goes away after one second what might that be a sign of?
-Normal response to patient as long as it goes away
If you get pain after using an ice test after 6 seconds do you have a problem?
-Yes
Is cold negative or positive pressure?
Negative
If the neurovascular bundle has been severed is the pulp tissue dead?
-Yes
What does a concussive effect do to the pulp?
-You get a few blood vessels that rupture
What causes a tooth to darken in a concussive effect?
-The heme from the blood going into the dentinal tubules
Why can’t pulp tissue heal itself?
- Limited blood supply
- Terminal end blood supply
How can you fix discoloration of a tooth that needs endo?
-Internal bleaching
