- correct electrolyte and acid based status
- remove waste products
- restore fluid balance
- improve symptoms

Benefits of dialysis
A
- improves uraemia symptoms (e.g. pruritis, tiredness, nausea)
- corrects fluid balance (less sob+oedema)
- avoid life threatening events (severe acidosis, hyperkalaemia)

Risks of dialysis
A
- complications (e.g. infection, hypotensions)

Benefits of kidney transplantation (5)
A
- better renal replacement
- costs less in the long term
- prolonged survival
- increased QoL
- improvement in metabolic disorders (e.g. anaemia, renal bone disease)

- immunosuppression (increased risk of infection and malignancy)
- older and sicker patients not eligible
- not a cure
- if transplant fails then worse

- when eGFR<10ml/min and benefits>risks
- eGFR<6ml/min and no reversible features
