Dopamine - Action (5)
A
- Dose dependent sympathomimemtic
- 1-2 mcg/kg/mins stimulate dopaminergic receptors > cerebral/renal/mesenteric vasodilation
- 2-10 mcg/kg/mins B1 effects (mainly inotropic) w/ alpha beginning (vasoconstriction)
- 10-20 mcg/kg/mins alpha effects ^renal/mesenteric/peripheral arterial vasoconstriction (increased SVR)
- 20+ mcg/kg/mins - alpha effects similar to norepi w/ peripheral vasoconstriction/tachycardia

A
- Cardiogenic shock
- Other Shock after adequate fluid resuscitation
- symptomatic bradycardia unresponsive to atropine or external PM

- Tachyarrhythmias
- HTN > MI
- Tissue necrosis w/ infiltration

Dopamine - Contraindications (4)

- Hypovolemia prior to fluid resuscitation
- Uncorrected tachyarrhythmias
- Monamine oxidase Inhibitors (MOI)
- Pheochromocytoma (precipate HTN crisis)

Dopamine - Precautions (4)
A
- Monitor VS/EKG
- Stop infusion if BP >90 mmHg
- Doesn’t work w/ Bicarb due to Alkaline deactivating dopamine
- avoid extravasation => tissue necrosis, inform RN
