Needle stick exposure. What are the 6 documents needed to be completed?

I’ll complete the “EPCR” and call contact BC. Let’s get you transported to the hospital after contacting BC and exposure control officer, Pam.

You will complete the “Royce log and SBCo dept of public health exposure documentation report”.

After you are treated, we will go back to the station and access the intranet to complete the “report of employee injury” and “DWC-1 workers comp forms”.

Complete the “sharps log” on the s-drive and I’ll complete my “logbook entry”.

Body fluid exposure documentation. What 5 forms do you need to ensure are completed?

Answer 2

After the exposure to body fluids we will contact the BC and Pam for direction on hospital destination.

You will arrive and fill out the “Royce log” and “SBCo public health exposure documentation report”.

We will head back to station intranet and fill out the report of “employee injury” and “DWC-1 form”. Lastly, I’ll make my “logbook” entry.
