What is the mode of action of Sulfonylureas?

A

Bind ATP-sensitive K+ channel on pancreatic beta cells. Causing electric potential over the membrane to become more positive.
This opens voltage-gated Ca2+ channels.
Rise in intracellular Ca2+ leads to fusion of insulin granulae with the cell membrane, and therefore increased secretion of (pro)insulin

What is the mode of action of Acarbose?

A

Inhibits alpha-glucosidase enzymes in the brush border of the small intestines and pancreatic alpha-amylase so less carbohydrates digested causing less glucose to be absorbed

What are the side effects of Acarbose?

A

Flatulence
Diarrhoea
Hepatitis can occur so need monitoring during treatment
If patient is hypo on Acarbose need to have monosaccharides as complex sugars are blocked by Acarbose
