Diverticulitis Flashcards
1
Q
Define diverticulosis
A
Presence of diverticula
(outpouchings of the colonic mucosa through colon wall)
2
Q
Define diverticula
A
Aquired outpouchings of colon mucosa and overlying connective tissue through the colonic wall
3
Q
Define diverticulitis
A
Diverticular inflammation +/- symptoms & signs
4
Q
Define diverticular disease
A
Diverticula associated with symptoms
5
Q
Draw a diverticula and a fistula
A
6
Q
What are the risk factors?
A
- 50+ years
- Obesity
- Smoking, NSAIDs, low-fibre diets
7
Q
What are the symptoms of diverticular disease?
A
If symptomatic;
- Intermittent LIF pain due to diverticular disease (IBD commonly co-exists)
- Constipation, bloating & rectal bleed (blood vessels are around diverticuli)
8
Q
What are the symptoms of acute diverticulitis?
A
- Rapid onset **LIF pain **& tenderness
- Bowel habit change
- Rectal bleeding
- Fever & tachycardia
- Neutrophil infiltration in colonic wall
9
Q
What are potential complications?
A
-
Mass/ abcess
- Persistent infection leading to mass
-
Peritonitis
- Perforation of abscess/ diverticula
-
Fistula
- Into vagina/ bladder
-
Stricture
- Chronic/ repetitive episodes causing fibrosis & colon narrowing
10
Q
What are the investigations?
A
CT Abdo
11
Q
What is the non-surgical treatment of diverticulitis?
A
- High fibre diet, high fluid intake, stool softeners (lowers intracolonic pressure)
- IV antibiotics
- Significant paracolic abscess drainage
12
Q
What is the surgical treatment of diverticulitis?
A
- Resection for;
- Acute inflammation failing to respond to meds
- Undrained paracolic sepsis
- Perforation
