Diverticulitis
Information on localize perforation of one or more diverticula with abscess formation
Diverticulitis causes/incidence
Higher incidence in those with low dietary fiber
More common in women than men
Diverticulitis signs/symptoms
A mile to moderate aching abdominal pain in lower left quadrant
Constipation or loose stools may be present
Nausea and vomiting
Diverticulitis physical exam
Low-grade fever
LLQ tenderness to palpation
Patients with free perforation present with a more dramatic picture or paritoneal signs
Diverticulitis lab/diagnostics
Mild to moderate leukocytosis Elevated ESR Stool heme positive in 25% of cases Sigmoidoscopy shows inflamed mucosa May consider CT scan to evaluate abscess **Plain abdominal films are obtained in all patients to look for evidence of free air
Diverticulitis inpatient management
NPO dependent on condition
IV fluids to maintain hydration
If significant G.I. bleeding is present, treat as outlined under PUD
20 to 30% of patients will require surgical management
