What is the diagnostic criteria for ADHD?
≥ 6 hyperactivity, impulsivity, or inattentiveness sx before 12 y/o for ≥ 6mo in ≥ 2 settings
How is ADHD treated?
stimulants + behavior modification
* 1st line: methylphenidate or amphetamine – most effective
* 2nd line: Atomoxetine or viloxazine – non-stimulants
When are sxs of autism usually recognized?
1-2 years old
What is the diagnostic criteria for autism spectrum disorder?
social communication/interaction deficits PLUS ≥ 2 restricted, repetitive patterns of behavior, interests or activities
Social (all):
* Deficits in social-emotional reciprocity
* Nonverbal communication behaviors
* Developing, maintaining, & understanding relationships
Behavioral (≥ 2):
* Stereotyped or repetitive motor movements, use of objects, or speech
* Insistence on sameness, inflexible adherence to routines, or ritualized patterns of verbal or nonverbal behavior
* Highly restricted, fixated interests that are abnormal in intensity or focus
* Hyper- or hyporeactivity to sensory input or unusual interest in sensory aspects of the environment
Sx must be present in the early developmental period & cause significant impairment
How is autism spectrum disorder treated?
Refer to autism specialists, speech & language pathologist, risperidone or paliperidone for irritability
What is conduct disorder?
- Aggression towards people, animals, & property, deceit & rule violation
- Often seen as the precursor to antisocial personality disorder
- MUST BE UNDER 18
What is the diagnostic criteria for conduct disorder?
persistent violation of basic rights of others or social norms PLUS ≥3 sxs w/in 12 mo AND ≥1 sx in last 6 mo
* Aggression to people/animals
* Destruction of property
* Deceitfulness or theft
* Serious rule violation
if ≥18 y/o, criteria not met for Antisocial Personality Disorder
How is conduct disorder treated?
CBT, family therapy, parent management training (PMT)
* Meds for comorbid sx/conditions
What is oppositional defiant disorder?
- MC in boys, more susceptible in kids w/ mood disorders, conduct disorder, & ADHD
- Unlike conduct disorder, children w/ ODD are not aggressive towards people/animals, dont destroy property, & dont show a pattern of theft or deceit
What is the diagnostic criteria for oppositional defiant disorder?
≥4 sxs occurring w/ ≥1 non-sibling in ≥6 months
* Angry/irritable mood: often loses temper, is touchy or easily annoyed, or is often angry & resentful.
* Argumentative/defiant behavior: breaks rules, blames others, argues w/ authority figures, deliberately annoys others
* Vindictive: has been spiteful/vindictive ≥2x in past 6mo
How is oppositional defiant disorder treated?
psychotherapy, CBT, family therapy +/- mood stabilizers, antipsychotics, & stimulants for comorbid conditions
