Disorders of calcium + bone metabolism Flashcards
What problem is indicated if calcium and phosphate are going in the same direction?
vitamin D problem
both low = vit D deficiency
both high = vit D excess
What problem is indicated if calcium and phosphate are going in opposite directions?
PTH problem
What does vitamin D deficiency in children put the children at risk of?
rickets
What is Vitamin D deficiency that affects the bones called in adults?
osteomalacia
What causes osteomalacia?
vitamin D deficiency
metabolic bone disease where there is inadequate mineralisation of the bony osteoid
Osteomalacia symptoms/signs
bone pain
myopathy
Looser’s zones - pseudofractures
biochemical abnormality
increased fracture risk
Rickets signs/symptoms
bowed legs
costochondral swelling
widened epiphyses at the wrists
myopathy
What is osteoporosis?
reduction in the amount of bony tissue within the skeleton
Why may osteoporosis arise?
failure to lay down sufficient bone during growth (peak bone mass)
earlier than usual onset of bone loss
increased rates of bone loss later in life
What T score would diagnose osteoporosis?
<-2.5
Osteoporosis treatments
lifestyle - stop smoking, do weight bearing exercise, reduce alcohol
bisphosphonates
vitamin D/Ca
denosumab
PTH derivative - Teriparatide
oestrogens - HRT
Bone profile results in primary hyperparathyroidism
high/N PTH
high calcium
low phosphate
high/N ALP
Vit D Normal
Bone profile results in rickets/osteomalacia
low vitamin D
low/normal Ca
low phosphate
high PTH
high ALP
Bone profile results in osteoporosis
Normal Ca
Normal Phosphate
Normal PTH
Normal ALP
Normal Vit D
