Diseases of the GI System Flashcards
What is dysphagia?

Dysphagia Symptoms

Where do patients complain of food getting stuck with dysphagia?

Diffuse Esophageal Spasm

Chronic Peptic Esophagitis “GERD”

Nissen Fundoplication
Bougie is inserted (dilator) to use as template to make a faux LES

Completed Nissan

Effect of Agents on LES Tone

Aspiration Pneumonitis
(Mendelson’s Syndrome)

Symptoms of Aspiration Pneumonitis
(Mendelson’s Syndrome)

Patient risks for aspiration

Treatment for preventing aspiration, after aspiration?

Hiatal hernia

- Decrease pH with citrate
- Reduce volume
- RSI

Esophageal Diverticula and symptoms associated with

Zenkers Diverticulum
Be careful with the NG tube, sometimes surgeon can assist placement

Mallory-Weiss Syndrome
Tumor induced

Peptic Ulcer Disease

Zollinger-Ellison Syndrome

Symptoms of Zollinger-Ellison Syndrome

Inflammatory Bowel Diseases
Will need to give steriod dose, or else you will be fighting hypotension

What Is the Role of Steroids in Septic Shock?

Peritonitis
(Do not see as much, maybe because we give Abx more now a days)
-Most will get a central line
-Volume and electrolyte status is critical

Treatment for Peritonitis

Prognosis of Peritonitis

Colorectal Cancer

Colorectal Cancer anesthetic management:

Pancreas Function

Pancreatitis

Causes of pancreatitis:

How does hard liquor differ from beer/wine with pancreatitis?

Acute Pancreatitis

Chronic Pancreatitis

Will have excess fat, 7-090% pain, diabetes, etc.

Sphincter of Oddi

Pharmacology of the Sphincter of Oddi
-
Glucagon- 0.1mg IV, will decrease sphincter of Oddi baseline pressure
- Can cause PONV, allergic reaction - N-butylscopolamine bromide (Buscopan)- 40 mg IV
- Inhibites spasms, takes 3-8min, may facilitate intubation of papilla - Nitroglycerin-1.2 mg sublingual

Pancreatic Cancer treatment

Whipple surgery

Carcinoid Tumors

Carcinoid Syndrome

Treatment of Carcinoid Syndrome
Octreotide or Lanreotide (somatostatin analogues)

Octreotide or Lanreotide (somatostatin analogues)

Anesthetic management for Carcinoid Syndrome
Cardiac workup
- Listen for heart murmor
- R sided heart failure
- Calcium deposits on valves
Complaining of wheezing and asthma
Check electrolytes/dehydration
Serotonin levels are high!

Prevention of Mediator Release for Carcinoid syndrome
Avoid histamine releasing drugs, maybe?

Treatment of a Carcinoic Crisis:

Management of anesthesia for Carcinoid anesthesia:

GI Procedures

Propofol for Sedation - Advantages

Propofol for sedation - Disadvantages

Endoscopic Retrograde Cholangiopancreatography (ERCP)
